Introduction

Tommy is a very active and curious toddler. His beautiful smile is a magnet that draws in meaningful connections. He loves engaging with others and is described as a “ray of sunshine”. Tommy has a tracheal tube, but it does not prevent him from walking, running, and playing with the aid of his yellow backpack which supports his G-tube feedings. He has a fraternal twin sister who he looks forward to spending time with.Tommy has not been able to experience a daycare or home setting due to his complex medical needs, he has been in a hospital setting since birth. Tommy has formed meaningful relationships with the staff at his medical facility. He is now thriving and developing communication skills through American Sign Language (ASL). Tommy’s perseverance and resilience have led to him being ready for discharge from the medical facility where he is currently placed.Tommy needs a loving family of any constellation, with or without other children in the home, experienced in supporting his medical needs. Potential families must be well organized, have excellent communication skills, have strong follow-through, and be willing to partner with his providers which includes home nursing. It is important that Tommy is able to preserve his relationship with his twin sister and older brother which includes monthly visits.
